Abstract:

To provide a cultural support to the administrative decision of discharging patients with a hospitalbased prescription (50% of the market price to be reimbursed by the NHS) for the first cycle of therapy, an individual patient oriented «posology sheet» has been adopted. The initiative is part of a broader project on the yield of strategy of pharmaceutical care which aims to favour continuity between hospital and ambulatory care.
