The 10 most important causes of hospital medication errors associated with the pharmaceutical industry and the Food and Drug Administration, including labeling, packaging, drug nomenclature, and device design, are listed; a second list of the 10 most common medication errors associated with prescribing and dispensing practices of nurses, physicians, and pharmacists is presented.
